Services and programs to help you at home and in the community
If you, or someone you love, need support to remain living in their own home, the Active Ageing and Diversity program is here to help.
Our program offers a range of home and community-based support services. This includes people who are frail, aged, or living with a disability, including their carers.
Getting approval to access home support services
Before you can access our services and programs you need to get approval through My Aged Care. My Aged Care will approve you so you can receive our services through the Commonwealth Home Support Programme (CHSP). The CHSP is a government-funded program to help you live independently.
The My Aged Care staff will help you through what services you need. Make sure you have your Medicare card ready when you call them.
