Services and programs to help you at home and in the community

If you, or someone you love need support to remain living independently in their own home, this page provides information on a range of home and community-based services that are available. This includes access to services for people who are frail, aged, or living with a disability, including their carers.

 

Services for residents over 65 years of age

Commonwealth Home Support Programme (CHSP)

The Commonwealth Home Support Programme (CHSP) helps senior Australians access entry-level support services to live independently and safely at home.

If you’re having trouble with everyday tasks and feel that a little support could improve your health and wellbeing, CHSP could be right for you. To be eligible for this program, you must be aged:

  • 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people), or
  • 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people) and on a low income, homeless, or at risk of being homeless.

Silver Chain Victoria was appointed by the Commonwealth Government as the new provider of CHSP services for Moorabool Shire residents over the age of 65 years, effective 1 July 2022.

  • Flexible Respite
  • Centre Based Respite
  • Social Support – Group
  • Social Support – Individual
  • Home and Property Maintenance

Services for residents under 65 years of age

Home and Community Care Program for Younger People (HACC-PYP)

The Home and Community Care Program for Younger People (HACC-PYP) provides support for younger people with disabilities whose capacity for independent living is at risk.

mecwacare was appointed by the Victorian State Government as the new provider of services to Moorabool residents under the age of 65 who require supports in the home or community, effective 1 July 2022.

  • Domestic Assistance
  • Personal Care
  • Social Support Activities
  • Respite
  • Home and Property Maintenance
  • Delivered Meals

 

How to access home support services

For Moorabool Shire residents aged over 65 years who need support in the home, approval through My Aged Care is required to access services. My Aged Care will assess your needs and identify the relevant aged care services available through the Commonwealth Home Support Programme (CHSP). CHSP is a government-funded program to help you live independently in your home.

The My Aged Care staff will help guide you to the services you may need. Make sure you have your Medicare card ready when you call them.

For residents under 65 years of age, supports can be accessed by contacting mecwacare directly on 1800 163 292 or 03 8573 4980. Alternatively you can email the Central Intake team on intake@mecwacare.org.au
